What should I bring to my visit?

Please be sure to bring your current insurance card. We will have you complete three forms (if you haven't before), or you can print them off, complete, and bring with you. The three necessary forms are our QMG Patient Registration form, the E-Prescribing form and the Notice of Privacy Practices.

Will I need to make a payment?

QMG will bill your insurance for your medical visit. If you have a copay for your medical visit, it will be bill along with any additional charges. If you do not have insurance, you will be asked to pay $88 at the time of your visit. Any additional charges will be bill. *Please credit or debit payments only.

Who should I call if I have a question regarding my QMG bill/account?

Our QMG Patient Service department is ready to help with any questions regarding your QMG account, a bill, etc. They can easily be reached by calling 217-222-6550, ext. 3444.

What days and times can I see the provider?

The Quincy Medical Group Nurse Practitioner will be available at John Wood on Tuesdays from 1pm to 3pm and Wednesdays from 9am to 11am. You are welcome to walk-in for an appointment or you can call the office to schedule an appointment. If you have a question for the provider outside of the normal hours, you can call the office at 217-222-6550, ext. 6633.

Can I see the provider if I have never been to Quincy Medical Group?

Certainly! Our Nurse Practitioner is happy to help you, even if you have never been to Quincy Medical Group. You are welcome to call the patient registration desk to pre-register for your visit. Call 217-222-6550, ext. 3650 if you would like tot pre-register for your visit or if you have changes to your information on your account.

Where is the on-site clinic located?

Room D004, the lower level hallway between the bookstore and the auditorium.
